Permission to Use Student Work

Dear Parent or Guardian:  

As your child’s student teacher for the next eight weeks, I am required to submit samples of student work for my final portfolio. 

I am asking for your permission to use, if necessary, samples of your child’s work.  If you agree, please complete and sign the form below for purposes of data collection for final evaluation of my student teaching performance. Your child's name will not appear on any of the documentations.
